Megacolon as my cat ages

Hi, my cat is 11 years old. He was diagnosed with megacolon at 1.5 years after a lot of constipation issues. He is on Royal Canin Fiber Response prescription food and it’s saved his life! His poops are huge and sometimes he struggles to get it out but he is never “constipated” unless he is sick and slows down eating. It’s been a great 10ish years.

Within the last 6 months, he started pooping outside the litter now and then. we thought it was behavioral, on days when both my husband and I are out of the house, but now it’s every single time he poops. I can tell he is trying to use the litter- we will find a small nugget in the litter and then the rest on the floor downstairs. It’s not “hard” when we pick it up, so I don’t think he’s constipated. It doesn’t seem defiant- he is always trying to cover it. He is still peeing in the litter.

We give him mirlax now and then to keep things soft. Fortunately, he didn’t give us a hard time about drinking lots of water, so the mirlax keeps him moving too.

His bloodwork is normal and our vet thinks it’s behavioral because I am pregnant with our first baby, but I am convinced his poor colon is just losing its functionality.

Thoughts? Is he going to need surgery? Id rather gross that bridge, if we have to, while he’s still younger and fit, than wait til he’s old.
