Montana Medicaid + ventilator-dependent adult transitioning from SNF to home — what benefits/resources should I be asking about?

My fiancé has a C3/C4 incomplete SCI and requires an invasive ventilator. We’re trying to transition him from a Montana SNF back into our home in Bozeman.
We’re working on Montana Medicaid, waiver programs and home-based care, but I’m trying to understand what we should specifically be requesting.

What Medicaid benefits/programs should I be asking about for:
-invasive ventilator + backup vent
-DME
-respiratory therapy
-private-duty nursing/home nursing
-personal care attendants
-Hoyer lift/hospital bed/shower equipment
transportation
-home modifications

Especially interested in hearing from anyone who has successfully transitioned a ventilator-dependent person from facility care to home using Medicaid.

New Chapter: What Advice Can You Give?

To cut a very long story short, I (f, 31) am engaged to a man (m, 40) with quadriplegia. He hasn't been home since the accident and we've been in healthcare hell for 2 years. We're at a turning point, he's at a SNF and being discharged *hopefully* in the beginning of August. I'm still looking for a DME company, secure transportation, made his PC appointment, and make sure we have all the equipment and accoutrements. Oh and keeping tabs on Medicaid approval to achieve some of those aforementioned check boxes. We're in an apartment

I'm writing because I need advice. If you were in my position, same situation, same age. What would you tell them? Real advice please, no "don't do it", irrelevant responses.
