how do you enjoy talking to people again?

recently i went to the dentist and i said something i think made the doctor get angry, she referred me with another doctor because of my comment. on the next appointment i tried to do small talk and i think i also said something stupid to the other doctor. i feel like when i say something people think i'm a weird piece of shit, and when i don't say anything people want me to talk. i've been isolated for a year and a half now, sometimes (ex)friends message me trying to tell me go out, but i feel i wouldn't encounter any pleasure in it and they would just think i'm a pathetic loser who can't even say anything.
