STJ vs real life experience - need tips for fill the gap

I'm doing some STJ practice before my exam during the summer, and I'm getting a bit frustrated.
I'm planning to apply to GEM, and I have been working full-time at a hospital (not disclosing the job title) for quite some time now.
And I'm getting half of the questions wrong in STJ, and it occasionally makes me shout, ‘Who actually works like this in a clinical setting!’
I wouldn't consider myself someone who works outside of guidelines, I'm more scared to do so. Also very up to date with all the NHS clinic staff trainings too.
The point is, there is such a big gap between my understanding of judging in a clinical environment and STJ, and I would be grateful if I could get advice from someone who studied STJ with a low first score or who was in a similar situation with me
