Did I really have cancer if it started as EIN (complex hyperplasia with atypia).

I had several heavy bleeding episodes in 2019 with negative endometrial biopsy, 2020 another heavy bleeding/flooding episode with another negative endometrial biopsy then had an ablation for the heavy bleeding (perimenopausal). Ablation failed but periods were lighter and manageable and I was rx 100 mgs progesterone nightly.

Another heavy bleeding/flooding episode in 2022 another negative endometrial biopsy. Lining was thin but there was a "blood lake" GYN's words next to the uterus and he said that blood would expel (it did and was horrendous and profuse).

2023/2024 periods were coming every 2/3 months as menopause was creeping in I was 50.

February 2024 started random spotting. Nothing major scant light pink after urinating. I figured another period was beginning. Then spotting stopped. A week later it was back. Then stopped. This happened off and on for 5 weeks before I made a GYN app thinking I needed more progesterone.

April 1 2024 GYN appt lining 11mm and endo biopsy not benign this time but "inconclusive needs further testing". GYN said looked like a polyp in the lining on Ultrasound but we need an operative hysteroscopy to be sure. I had had singular small benign polyps 3 times prior in 2007, 2009 and 2020 that were all removed and all benign. My estrogen was always high. I was fit and not heavy and non PCOS and had had children. Didn't make sense. Didn't drink or eat a lot of meat dairy or sugar.

All Pap smears clear always.

April 22 2024 Had an operative hysteroscopy, with DNC and polypectomy.

April 23, 2024 2 years ago yesterday I got those hysteroscopy results sent to my MyChart and read that I had complex hyperplasia with aypia/EIN and one small polyp 4mmx4mm that contained adenocarcinoma.

Negative for Lynch. No p53 or PTEN.

My question is; does all endometrial cancer start from EIN? Could those biopsies I had in 2020 and 2022 have not picked up the EIN and said benign even tho not benign.

Upon hysterectomy the pathology was no cancer was present in the uterus. 0% invasion to the uterine wall as the polyp sat atop the uterus and it was removed entirely with the hysteroscopy. Lymph nodes clear. 3 lymph removed on left side clear and Sentinel node removed on right and clear, pelvic wash was clear. No LVSI.

The hysterectomy pathology showed adenomyosis and EIN.

I ask my oncologist often was I stage 0 and he says NO. You were Grade 1A, Stage 1a.

••How do other endometrial cancers start if they don't also start with EIN and are EIN endometrial cancers different than other endometrial cancers?
