Bowel Obstruction but IBS-D?

If i don’t take my meds, i will poop 100 times. If i take them, i still have a few bowel movements in the morning and maybe another during the day later on. My main most debilitating symptom rn is the spasms/tenesmus. I had a proctogram and the results were that i have some sort of bowel obstruction. However, i poop a lot so i don’t understand how im meant to battle this. Don’t take my meds and go bathroom all day, take my meds but have spasms and tenesmus from some sort of bowel obstruction. What am i meant to do to help? I’m so lost and defeated. We aren’t sure if this is some pelvic floor issue/dysfunction so i’ve been leaning towards trying botox in my pelvic floor and see if it helps these symptoms. I just don’t know what to do anymore. Please help

No antispasmodics helps my debilitating spasms. Im currently on antidepressants already as well. Would trying an antipsychotic to treat my hypersensitive central nervous system and anxiety thus helping my spasms be the best next route?

I also already have a sacral nerve stimulator (SNS) which has done absolutely nothing for me. I’m thinking my issue is something to do with hypersensitive nerves in my bowel so if SNS hasn’t worked, is there another nerve stimulator of sorts i should try?

Idk i’m just stumped and defeated

The main most debilitating symptom i have is intestinal spasms. These spasms are sharp and are constantly coming and going 24/7. They feel like sharp spasms of rectal urgency, or tenesmus, like i’m about to really crap my pants, but these spasms rarely are actual bowel movements or sometimes end up being gas which i can pass to relieve. However, if not a bowel movement or gas, these spasms just continue, and it feels like i have anxiety cramps and a nervous stomach all day everyday. I really have little to no relief for these spasms even after trying all first-line and second-line treatments i can think of. I’m currently taking amitriptyline, paroxetine (or paxil), pregabalin, and all the antispasmodics available to me, such as mebeverine or buscapan, peppermint oil etc and Simethicone. I also even have a sacral nerve stimulator (which did nothing for my stomach) and currently have been using a TENS machine for the last few days which has helped me somewhat process the pain differently and more tolerantly, but it is still unbearable as it is that I can’t even work at the moment.

My specialist thinks this could be some sort of pelvic floor dysfunction perhaps that is manifesting into IBS symptoms or overlapping with IBS. I’m also coeliac and lactose intolerant but strictly adhere to a restricted diet and since discovering my coeliac disease over 4-5 years ago, colonoscopies have since seen the damage to my intestinal lining healed. However, doctors and I are truely and utterly stumped.

I do currently smoke cigarettes which i’ve been meaning to stop but i’ve had these symptoms for as long as i can remember, even before the smoking. Anyone quit nicotine or tobacco and found their ibs spasms to drastically improve?

i’m 22f and also exercise every day, i don’t understand what else i can do besides quit smoking, and if i quit and nothing changes, what else is there I can do? Anyone else have any out of the box ideas or treatments that have helped their spasms?

Thank you for any help as i’m truely and utterly defeated and i feel as i have lost my life to this stupid disorder.
