Immediate relief?

Hi all - I've dealt with anal fissures for the last 15 years but only recently (January) had the spasms. I have Crohn's so initially I started with my Gastro - she eventually said this fissure was chronic and out of her scope of care and referred me to a colorectal surgeon. I see him on the 21st of this month but honestly am not sure I can make it that long. Unfortunately for me, most of my bowel movements happen in the early evening which mean I'm up most of the night (until 2-3 am) with spasms. I am severely lacking in sleep and with two small children I can't really adjust my daily schedule to make up for it. Thank God I work from home or else I would have been on leave this entire time because there's no way I would be able to go into the office. I have a bidet, I use a squatty potty and wipes, I use Calmoseptene, do not push hard bowel movements out (I'm actually not constipated at all), I sit on a heating pad all day and take frequent warm baths. All of that only helps temporarily and there is nothing I have found that helps immediately stop it when I need to sleep in the night. I have tried various Yoga poses like Child pose, baby, etc. I have massaged the area. I am not able to get even a q-tip in easily so massaging internally is out of the question until I get this thing healed. My gastro gave me nitroglycerin which does not seem to work on me at all. I have four very angry sentinel tags and some internal hemorrhoids as well. Does anyone have anything that can help stop the spasms so I can sleep? Tylenol and Ibuprofen do not do anything and while warmth helps - it only helps when I directly sit on it or in it and as soon as I get off the heat it starts again. Thank you! I'm desperate for sleep!
