I need an advice

Hi, im currently on my first run with my Jorgrim, 1H axe + shield. My problem is I can easily do tier 4 contracts but when i want to do tier 5 dungeons it's impossible, i get clapped everytime, too much enemies and max danger ones. I'm trying to do Fort Gray Falcon. Can someone more experienced give me an advice? I would be so grateful :). I upload images of my character stats and build (all tier V items except chest armor).
