My brake rotor keeps coming loose no matter how many times i tighten the retaining screw and torque down the spacers and wheels it keeps coming loose and causing an annoying noise while driving. Not sure what the issue is. Anyone have any ideas?

I know some trims came with vents in the back of the center console for the rear seats but mine has the charging ports. Would it be possible to swap it out? and would it be a pain in the ass if so?
