I’ve been on 50 mg spironolactone since March 2026. I added 20 mg isotretinoin on April because my acne wasn’t improving. I know spironolactone can take time to work, but my breakouts were severe. I also read on this subreddit, this combo works better.

My bloodwork showed high androgen levels, and new pimples kept appearing. It’s now been a week since I increased spironolactone to 100 mg and lowered isotretinoin to 10 mg. I’m still getting about 2-3 cystic pimples a week.

Is this due to the dosage change? Could it be purging? Am I doing something wrong? For a reference, I just recently got married so this acne really bothers me as I have to wake up every morning and cover it up.
