What should my next step be in sterilization?

I've been thinking about getting sterilized for a few years now (21F) I always just assumed I had to wait due to people always saying they won't do any procedures unless you're this age or have this many kids.

That was until I found this subreddit. Now that I am aware I can have tubes removed despite my age, I have been researching the different procedures and trying to be better informed before consulting my OBGYN.

I had an appointment scheduled with her last month, but they cancelled it on their app. I attempted to call, as well as reschedule through the app, but I can't seem to get through to them. Because of this I am no longer getting my Birth control.

I am absolutely terrified of having kids. I'm sure everyone here can relate when I say I'm trying not to explain why I don't want kids. It's still a foreign concept to me that no one needs an explanation past I don't want any.

I decided I just want my tubes out, so I never have to worry about it whatsoever.

So, I guess some questions I have are, Do I need to have a regular OBGYN that refers me to a specialist? If so, how do I find a reasonable OBGYN in my area that is willing to refer me? If not, can I just find a doctor near me on the Friendly doctor list and make an appointment with them? If that's the case, can I just call and let them know over the phone what my intentions are for the appointment or do I need to get a regular checkup and mention it during that?

Sorry, this is so long, but more info that may or may not be relevant is I am currently engaged but not married. Does my fiancé need to be there? or a parent? I also am located in Ohio.

Again, I apologize for the long post and the potential "well obviously" questions. I just want to make sure I am taking the correct steps to not potentially hinder myself, especially with the OBGYN drama. Thank you!
