Marginal keratitis - bacterial or HSV?
Hey everyone, 28M here. Was diagnosed with marginal keratitis in my right eye about 1.5 weeks ago and getting conflicting opinions from doctors.
What happened:
Woke up one day with watery discharge, mild pain, light sensitivity and foreign body feeling in my right eye. Went to multiple doctors and finally a cornea specialist 2-3 days after the onset - found two white infiltrates at the edge of my cornea with an ulcer at the corneal edge.
Current treatment:
Gatilox drops every 2 hours, Loteprednol twice daily, Moxifloxacin ointment at bedtime, Doxycycline capsule, Hylosoft lubricant drops and Famciclovir antivirals.After week in, doctor confirmed no epithelial defect at last visit (was present in the first visit)
The confusion:
• One doctor says pattern doesn’t look like HSV
• Another (cornea specialist) says watery discharge points viral
• I carry HSV-1, get cold sores occasionally
• No blepharitis ever - clean lid margins
• Pain was surprisingly mild
• I did notice 1 small clear bump on my lower eyelid - looked like MGD based on google pics but ofc i am not sure
My questions:
1. Does this sound bacterial or HSV to anyone who’s experienced this? Both doctors mentioned different things and I want to know root cause since I am quite worried about recurrence
2. Anyone had HSV keratitis with no dendritic pattern on early examination and an epithelial defect
