Granddad and Medical Decisions

Good day, friends. My family and I need your advice. My sweet granddad has dementia that has rapidly progressed. His doctor did not properly monitor the progression and now my granddad is still considered competent to make decisions about healthcare that he doesn’t remember that he needs. An example of this would be the catheter he kept pulling out because he didn’t know what it was nor did he realize it was inserted.
We are trying to find ways to help him and be there for him to navigate his current needs. Does keeping a journal written or recorded help? Or a binder of work/needs/conversations? Especially if all have bought into the idea.
