Low supply

I’m looking for insight from anyone who has dealt with very low milk supply despite doing “all the things.”
My baby is about 2.5 months old. I’ve triple fed for 8 weeks and worked with multiple IBCLCs (8 total at this point). I only produce around 1 oz total per pumping session from both breasts combined. I also tried supplements, water, electrolytes and Reglan
My baby transfers roughly 40 mL during a breastfeeding session based on weighted feeds, so we’ve been combo feeding with formula.
A few things I’m wondering about:
- When I hand express or press behind my areola, milk often shoots out in streams, but my baby and pump don’t seem able to empty those areas effectively.
- Milk seems to come from only a few nipple pores, and it’s always been that way.
- I usually only notice one letdown when pumping.
- My nipples don’t seem obviously clogged, but I wonder if there could be some kind of duct issue.
- I’ve recently started metformin Type 2 diabetic here and am curious whether anyone noticed a supply increase from it and how long it took.
- gotten lab work done. No thyroid issues.
Has anyone had a similar situation? Were you eventually able to identify a cause (hormonal issues, insufficient glandular tissue, pump fit, duct issues, etc.)?
At this point I’m mostly looking for experiences from people who had severe low supply despite intensive pumping and lactation support.
