Examples of how to write notes

So, weird question, but I feel like I’m doing my notes wrong and my supervisor isn’t correcting them. I’ve realized that every counselor at the practice is writing in wildly different styles and is interpreting the fields in TherapyNotes for example totally differently, which is making me doubt if I’m doing it right.

I don’t have a problem writing notes within 24 hours, and I know my notes are too detailed honestly but I’m graduating at the end of this semester and freaking out that I may be doing them wrong.

Does anyone have any examples of notes, especially if they’ve used TherapyNotes’ fields before?

Also, everyone at our practice takes notes during session - I’ve never had a client complain about it, but is that abnormal??

Confusion about toothpastes

Please help - my gums are severely inflamed and my teeth are just crumbling. I did 3 root canals and 3 crowns in January, and yesterday a piece of my top tooth just fell out, I guess there was a cavity there that they couldn’t repair because the tooth is mostly held together with fillings. I’m absolutely freaking out, I can’t seem to stop the spread and my gum issues are making things worst. I had a deep cleaning last year, followed by 3 month cleanings but had to stop at the end of my pregnancy because I can’t lay flat without passing out.

I started researching toothpastes and I’m just so confused, I see livfresh and hydroxyapatite toothpastes and some say remineralizing, some say they help gum inflammation and prevent plaque from forming. I really have no idea which to choose, which one actually works or helps.

Please I need your recommendations - I absolutely do not care about the cost, I’m 10 k deep into the dentist just in the last 5 months with no end in sight, toothpaste isn’t going to be the thing that bankrupts me at this point

I have a heavy adolescent case load and I just feel like the way I start my sessions isn’t resonating with them. If I ask how the last week has been, everyone’s answer is always one word: fine.

Anybody got any suggestions for opening the session that’s more engaging?
