Need some reassurance from fellow DOR IVF people 😭

I’m 36 with AMH 0.39 ng/mL, FSH around 11.5, and my baseline AFC in March was around 5. I’m currently doing my first IVF cycle with a luteal estrogen priming antagonist protocol.

Timeline:
-Started Estradiol 7 days before my period
-Started stims (Follistim + low-dose hCG) on CD3
-Had my first monitoring appointment today on CD7 (about stim day 4)

Today’s ultrasound showed:
6 follicles on the left ovary
1 follicle on the right
All still under 10 mm

My estradiol today was 102 and the clinic said my bloodwork looked normal and “where they want it to be.”
Logically I know this is still early and follicles are supposed to still be small at this stage, especially with DOR, but emotionally I keep going back and forth between “okay this is encouraging” and “what if they stop growing?” 😭

I think because I mentally prepared myself for low numbers, I’m actually happy they saw around 7 follicles responding, but now I’m anxious about the next step and hoping they grow more by my Saturday monitoring appointment.

Would love to hear from anyone with similar numbers or DOR who had slow/steady starts and how things progressed from here.

DOR and PMOS (PCOS)

Hi everyone! I need to rant for a second because I’ve been super confused and this has been on my mind all day. I’m starting my first IVF cycle and I’m wondering if anyone else has experienced something similar.

I’m 36yr old and have been trying to conceive for about 1.5 years. I had a miscarriage and during the fertility workup I was diagnosed with DOR. My AMH was .39 ng/mL and my FSH was around 11.5. My AFC at baseline was about 5. What has really messed with my head is that I was diagnosed with PCOS at 26yr old. Multiple ultrasounds over the years showed the “string/thread of pearls” appearance, so I always associated myself with PCOS and thought that usually meant HIGH AMH or lots of follicles. So getting hit with a low AMH diagnosis honestly shocked me.

My protocol is luteal estrogen priming antagonist protocol. I started Estradiol 7 days before my expected period and starting stim injections tomorrow on CD 3 with Follistim + low dose HCG, and adding Ganirelix later in the cycle.

Has anyone else had a history of PCOS but later ended up with low AMH/DOR? If you had similar numbers, how did your first retrieval go?
