How do you know which marketing actually brings in patients?

I’ve been trying to figure out how other practices track where new patients are really coming from. It’s easy enough to see clicks, calls and form fills, but that still doesn’t tell you which channels are producing patients who actually book and show up.
I’m curious how other practices handle this. Do you track everything in one place, use separate tools for calls and website leads, or just rely on monthly numbers from your marketing agency? And which metric do you care about most: cost per lead, cost per appointment, conversion rate, or revenue?
