I am an NP that works for a program called PACE. It’s very different than traditional primary care and a lot of how we function is regulated by CMS. Part of my role is to deny authorization to specialists that I no longer feel follow up is necessary. These are my patients and my liability, super stable patients that I will continue to follow and manage. So I get an auth request I deny it and write my reason usually supported by guidelines and literature, I am able to continue care and will refer back prn. I like what I do but I hate the quality department because they come up with all these stupid rules and say “per cms guidelines.” So they push more admin work on everyone, including pcps. I am being told that for every denial it is my responsibility to write up the denial letter that will be sent to the patient. They have a template for us, but I still have to create the letter and again explain why I am denying. And if the patient speaks Spanish I need to make a copy in Spanish. I feel that this can be delegated and if quality requires it quality can do it, copy and paste what I have already written in the chart. I feel this work is demeaning and we already have a lot of charting/ documentation responsibilities. Can I refuse to do this task? I talked to my sister in law who does billing and she said no way would they make their providers do this. I want to refuse so badly and don’t really care if I get written up and sent to HR. I am just so frustrated with these customer service tasks that are being put on us and quality telling us “this is cms guidelines so you have to do it”
