Good morning, I’m moving abroad and filling out my Marriage License info online. Pero ilalagay ko daw dapat ang last name at birth ko dun.

Sa PSA ko, I was born in 2000 but may nakalagay sa gilid (example name) na “Taylor Alison and “the surname of the child is hereby changed from Alison to Swift on April 26, 2010. The child shall be known as Taylor Alison Swift pursuant to R.A. 9255.”

The question is, should I put “Taylor Alison” as my last name at birth or yung “Taylor Alison Swift” na naging effective 2010?
