Certificado de derecho a residir con carácter permanente como ciudadano de la Unión Europea

Hello, I've been living in Spain for 10 years and I have the "green NIE" (CUE). I have read about the possibility after 5 years of getting a permanent certificate, from what I understand it's again a green card but states explicitly that I reside permanently in Spain.

However, I'm confused about the necessary documents and the process. On the Policia website it is mentioned the EX-18 form, an ID national document, paid fee, and what seems to be a CUE of at least 5 years. However, at other places I see more requirements and I'm not sure if I need to prove again the conditions for getting the green NIE the first time. On the Extranjería website there's no information at all about a separate procedure from the one for obtaining CUE.

Has anyone gone through that process after having resides in Spain for more than 5 years? Can you share your experience?
