Looking for a recommendation on a State Farm agent in the Nampa, Caldwell area. Who do you like/ work well with? Who should I stay away from?

We have a almost 15yr old standard poodle that is showing rapid decline and would like to have a mobile vet handy just in case that time comes.

Anyone have any recommendations on a vet in the Nampa/Caldwell/Meridian area that can come to our house to help us say goodbye when the time comes?
