Keep going in Berlin or try Prague

I’m in a really frustrating place and could really use some advice to help us make a decision.

My husband (38M) and I (38F) had been trying to conceive for a year when we found out that my husband has obstructive azoospermia due to a genetic issue. He underwent a TESE procedure, and the doctors said the results couldn’t have been better.

We then started ICSI at a clinic in Berlin and have had two cycles so far. Although my blood tests and ultrasound results were optimal, they retrieved only 3 eggs in the first cycle, all of which were mature. From those, they were able to transfer 2 embryos (1 blastocyst and 1 morula), but unfortunately, neither implanted.

In the second cycle, they retrieved 5 eggs, again all mature, but were only able to transfer 2 day-3 embryos. Unfortunately, this cycle also failed.

Given that my AMH is 2.0, I feel really disappointed that they were able to retrieve so few oocytes. They also found an endometrioma on my right ovary, which seems to be contributing less overall.

Now we have only one attempt left that will be covered by our insurance. When I asked the clinic whether they would change anything in the protocol or treatment for the next cycle, they couldn’t give me a confident answer.

It feels like we’re not getting personalised treatment at all. Different doctors see us at every appointment, and they often read through our file during the appointment itself, which is a red flag for me.

We’re now speaking to a couple of clinics in Prague to get a second opinion.

Has anyone been in a similar situation? What did you change, and what ultimately worked for you? I’d really appreciate hearing about your experiences, especially if you had low egg retrieval numbers despite having a relatively good AMH.
