Child Passport w/o court order?
Hi, please no judgment. I’m planning to apply for my child’s passport and was wondering if anyone has had success using only the DS-5525 form.
Long story short, the father listed on the birth certificate has had no contact since my child was born. The relationship involved physical and emotional abuse, and I do not know his current whereabouts or contact information. At one point, Michigan Child Support had an open case trying to locate him, but they were unsuccessful and eventually closed the case after I disclosed the abuse situation.
I was previously able to legally change my child’s name through the court, but I never filed for sole custody or a passport order. I am currently trying to get legal help to pursue that.
I’m just wondering if anyone has had a passport approved with a DS-5525 alone in a similar situation, or if the State Department requested additional documents afterward. Any insight or experiences would really help. Thank you.
