Plot hole
I mean I don't even understand how the writing staff came up with this, the most straight edge and law abiding people I know (at the most dabble in domestic violence, and not because dinner is cold, I'm talking disrespect, not being in the mood and backtalk), would've at least take it for pain management.
If it's not revealed later this is self-hating and extremely masochistic individual this series belongs in the "Fantasy" category. For a show that is so historically correct (I will come back to this), this really put me off. As in took me by surprise and gave me the feeling of living in a Salvador Dali painting, surreal and anxious, the Doctor and Nurse acting all normal didn't help. Thankfully the wife seemed super stressed and very uncomfortable in the whole scene, like she didn't even understand what to say/do. Maybe that is supposed to be the audience "self insert". Bravo if so, haven't seen this type of layering in cinematic effects before.
