When is it time to say goodbye?

My puppy was born with a heart defect. Dual chambered right ventricle. He's been actively in CHF for his whole life.

We had surgery to correct it, and he was better for 6 months. The post surgery checkup said his heart was normal and other than taking pills, he would expect a normal healthy life.

About 1 month ago, he went on a decline. His energy is dropped, and he's constantly fighting with ascites. We keep getting liters of fluid drained but it comes back after a week.

He has a cardiology check up in a month, but we keep worrying we're going to wait too long, and he's going to be in pain or uncomfortable. He just turned 1 year old and already beat the odds pretty significantly.

We would rather he go as happy and healthy as possible, but are scared of being preemptive when he could bounce back.

We're talking to our vet about saying goodbye, but I would love anyone else's input who have had to say goodbye while uncertain, and how you know when it's time.
