I have lost my I'd card during exams and due to rush I forgot to visit tnp to complaint about it . Can anyone pls send me their copy of fir or the request for duplicate id card wala page so that I can also apply for it!!

I have lost my I'd card nearly 2 months ago and not reported it to tnp .. can I apply for a new I'd card via ums id card request?? Does it works or I have to go to tnp? If yes then please guide me what should I do ?
