What am I doing wrong? Placed the legendaries in the improved dismantling bench but no legendary frags are forthcoming. I assume i am doing something wrong-

Periodically my character teleports into the foundations the transporter sits on. I have to keep dismantling the things to get out.

This happening to anyone else?
