Hey so I messed up big. I haven’t downloaded the medical examination form and I have my appointment booked for tommorow. 😭😔

I have been struggling with trying to relogin to in4sit / matriculation portal but I think they closed the portal after 30th June and I haven’t been able to download my form… (ik I should’ve downloaded it immediately back in June 😭😭 I just didn’t expect the matriculation portal to close in June)

I was wondering if anybody could send me their examination form to use as a template, and id just photoshop/ edit stuff like name and NRIC 😔😭😭

Either that or any direction on who to contact would be fantastic… I’ve emailed the matriculation email several times over the past week but no reply 🥲

Edit: I am in accountancy
