Help Me With Ira

So I've been married to Ira for sometime (around 5-6 years in-game), and I've had with her 2 daughters.

And I recently wanted to have more, but, I'd spend weeks in a settlement with her in my party, and she wouldn't be pregnant, please help, why is that?!

Is it because our relation is 21? Or is it because I have my sister in my party as well who Ira sees as an enemy?

or is there a hidden factor, or am I just super duper unlucky?
