Stratagems that don't work

Here's what I experienced so far, I can't verify for older ones so feel free to add the bugs you've seen.

Come prepared : No stimms spawn, but 10 stimms on start.
(Only gives you 1 stimm on start, nothing more)

Measured mercy : Get 5 relics at the START, if someone dies, fail.
(Only gives you 1 relic at the start nothing more)

Temporal Boosts : Power up spheres spawn.
(Really clunky on if they register when you pick them up)

Hallowed Relic : Relic that heals and boosts damage, fails mission if lost
(Only heals players present at the start of the mission)

Shockwave plating : Regen armor on explosions
(No clue if this works or not but I've never seen it do anything)

Great responsibility : 1 Player has more health, if they die, fail
(Not a bug but there's no way to tell if your HP is increased from the HUD)

Squad Unity (Might be fixed) : If split up, no armor or contested health
(People mentioned it sometimes only working if you split and then come back close)

I feel like this is a lot of bugged or poorly made stratagems and I'm wondering how it can be so bad since its not like they have stopped making content for the game or abandoned it.

Do our angels know how to read the codex ?

Another squad of the emperor's angels was deployed on a weekly mission against the forces of chaos. I assume they are briefed like everyone on the mission and yet it seems every single one of them except for their shield bearer seems to have read the mission brief and the "complications" they would be dealing with.

Now I would never be one to doubt the emperor's wisdom as to the strength of his forces, but doubt has been sowed in me and I have begun to think that while most of them seem strong of will and body, some may be made weak of mind or at least not intelligent enough to read their mission briefing before engaging the enemy.

(In all seriousness, why does it seem that every week 90% of players spamming the stratagem don't even read what the modifiers do. I'm coming home from a hard day, I don't need to be teaching grown adults how to read 3 simple sentences)
