How did you know you needed surgery?

I’ve had TMJ for nearly 20 years (muscle pain more than joint issues at least as of my last X-ray about 6 years ago, Botox, saline and lidocaine injections, Invisalign, and lots of PT), sleep apnea diagnosis about 5 years ago, and have always had a weak chin. But none of the doctors ever mentioned surgery. Who first told you you should consider it? Does the fact no one said anything mean I’m not a candidate? And how did everyone here come to learn the terms / jargon and understand the universe of options out there? I didn’t even know sleep apnea was related to jaw issues until I came to this forum! I’m worried if I just go to a surgeon for a consult they’ll of course tell me I need it.

ETA: I also have/had an open bite- Invisalign helped but not fully. I used to be able to stick my tongue through my teeth on the sides with my molars touching in the back. Also I’m in NYC for context and 40 yo female

Surgical options - can I avoid a TT?

I have an upper and lower DR and an umbilical hernia. I am 5’3” 135 pounds with an athletic build. I have great muscle control when I engage my core but have no resting tone. The first three pictures below are from when I am relaxed and the second three pictures are from when I engage my core. The DR causes back pain and poor posture (Which leads to neck pain!)

I would like to avoid a tummy tuck because of the additional risk of the tightening, extra recovery, the drains, and I also don’t want a very snatched look. I have no stretch marks – any lines you see are from my pants being too tight lol. I definitely have excess fat that I’d want removed. My skin seems to have good elasticity and I don’t mind a little bit of wrinkling when I sit or bend over etc. Will I regret not getting the full tummy tuck? Has anyone with my body type had just the DR repair? I would love to see before and afters of someone who looks like me!
