Is this high school transcript format match for GKS-U requirement ?

This is my high school transcript, can any one suggest me that it's right for GKS or not , do I have to provide additional documents to explain it. And I made this by myself since my school located in remote area so they don't know how to do it .so I only plan to get stamp and signature from my Principal if this format ok for GKS requirement.

Your answer will be really helpful for me.

Official certificate

Any Cambodian's people here plan to apply GKS-U this year ,how y'all do to get an original certificate (graduated high school last year ) or have anyone here who has been selected as a GKS-U scholar how had you done to get your official certificate on time ?
