Has anyone ever received their pip assessment back with factual errors. It says I have no mental health conditions but I was diagnosed by a psychiatrist for OCD, ADHD, depression and anxiety. It say I can prepare food unaided when I said I cannot prepare food unless my partner does it. It also says I have no medical input for fainting, but I am diagnosed by a cardiologist with POTs, which I provided them with. I also scored 0 on everything?

I understand scoring may not always be what you think. But to claim I don’t have diagnoses that I actually have, and have been formally diagnosed with by medical professionals ? Any advice would be appreciated.
