Is progesterone therapy still used for some PFS patients?

(Im post AI) I personally never crash and just have sexual side effects, my only bad reactions to any intervention is slightly lower libido that is already super low let’s say from 5% to 1%. HCG (all my libido, random erections, pleasure all symptoms resolved within 3h post injection but stoped working after 4months)worked perfect and 2years later my erections are still improved from it. steroids or supplements don’t do shit so my guess is that I’m neurosteroid deficient since no other stuff even changes how I feel. (except cdg which I used for 80days at 3g a day but it only gave me a bit better libido for 2 weeks). My question is, is progesterone therapy worth trying with Sommer or is it out of the window with the new model?

Tellogen evluvium still goin on 5 years after crashing from anstrazole.

Hi guys, before crashing my hair was super thick, norwood 0. after I crashed I started rapidly loosing hair in a very even manner just like tellogen effluvium. It slowed down but never stopped, I lost almost half my hair density (no norwood, it’s all even)

My most obvious labs look normal. I have no food deficiencies like Iron, zinc or d3. Any idea what could it be? what to check? Male, 26yrs old.

  • Total Testosterone: ~486 ng/dL
  • LH: ~2.8 IU/L
  • Estradiol (E2): ~36 pg/mL
  • SHBG: ~27 nmol/L
  • DHT: ~248 pg/mL
  • DHEA-S: ~248 µg/dL
  • Prolactin: mid-range
  • Progesterone: mid-range
  • thanks guys 🫶🏻

I think PFS/PSSD/PAI is GABA-A and glutamate related.

Did any one else get brain zaps (like on ssri), muscle spasm(like benzo withdrawal) and insomnia(gaba-a) when crashed/crashing? My muscle spasms were extreme when crashed on anastrazole(which lowers gaba-a) they were so strong I could not straight out my legs. Maybe GABA-A and glutamate crashed and the system upregulated so much it’s now too much for it to handle? Or maybe it’s still too low?
I’m not sure , just wondering. Let me know your thoughts.

Castration trial on Post anastrazole syndrome

Disclaimer: this is not medical advice, I got my doctor to try the castration method with me. 6 weeks of relugolix with 2weeks of hydrocortisone (same time starting after 2weeks of relugolix).
I used AI to write the summary about me below but I entered all the data myself.
I will update you guys every two weeks.
Now I got only sexual symptoms like PFS but first 6 month was so many symptoms I don’t have the patience to write them out.
—————- Summary starts:
Patient: 26-year-old male
Crash: 5 years ago — anastrozole + exogenous testosterone (anabolic steroids), alongside ketoconazole shampoo and minoxidil. Not finasteride-triggered. Supraphysiological T converted to DHT via fully intact 5-alpha reductase; anastrozole blocked the aromatase escape valve; genetically impaired clearance system overwhelmed by the substrate load. Result: zero libido for 5 years, diffuse frontal hairline thinning (crown/mid-scalp/sides preserved, non-Norwood pattern, follicles present but miniaturized), watery/reduced semen quality, standard hormone panels reported as normal.
Genetics (consumer SNP array):
SLCO1B1 — homozygous *5 (rs4149056 TT) — most severe finding, clinically validated
UGT2B17 — likely heterozygous deletion (rs4440295 no-call)
UGT2B15 — multiple indels
UGT2B7 — deletion indels
ABCC2 — rs717620 CC, reduced expression (Powers flags this as a top-tier PFS gene independently)
SLCO1B3 — heterozygous, partial backup capacity
COMT — homozygous Met/Met (rs4680 AA) — slow, clinically validated
DBH — heterozygous at two functional sites
Unrelated but noted: APOE e3/e4, both Lp(a) risk tag SNPs, heterozygous MTHFR (both C677T and A1298C)
Labs: Progesterone recalled as medium-range (older test, not yet repeated). No formal 3α-ADG, Dutch Complete, DOC/corticosterone, or bile acid panel yet — ordering as baseline before relugolix.
What worked:
HCG 1500 IU — complete symptomatic normalization, but only ~3 days per injection, tracking HCG's pharmacokinetics exactly; required reinjection every 3 days; effective for ~4 months total before it stopped working; second attempt 6 months later — no effect at all
Calcium-D-glucarate, 2 weeks (tried after the SERM course) — temporary libido increase and stronger erections; ejaculate volume/force improved and persisted even after the libido effect faded
What didn't work / made things worse:
Testosterone injections — zero effect on libido or overall feeling
Anabolic steroids, DHEA, pregnenolone — no meaningful effect
Clomid + tamoxifen, 45 days — libido significantly worse, persisting 1+ year and ongoing
Current plan: Relugolix (chemical castration 6weeks) with a 2-week hydrocortisone course (30mg/day, split 3×10mg),for glucocorticoid metabolite clearance; CDG as clearance-support adjuncts; creatineongoing for COMT support.
I never crash I always endup with slightly lower libido that comes to the low baseline after some time.
u/drwillpowers Thank you for everything you do. Do your patients use hydrocortisone in the beggining mid or end of castration?

Any way to masculinize born male other than steroids?

A lot of trans people on the sub, I’m interested in masculizing somebody that is already male at birth. Is steroids the only option or does DrPowers know something counterintuitive in that regard too?

Got out of debt wants a motorcycle license.

Im 26 just got out of debt because I made some business mistakes that cost me 3years of my life and 30k usd. I save around 660usd monthly. I been very frugal the last 3years. I’d love to ride a motorcycle. Should I get the license next month that costs 1000 and later a bike that costs 2000usd or should I save that money and buy it later? I got a old car but it drives and I don’t use it daily anyway.

Finding your weird metabolite

Hello :)
To know when to end castration we need to monitor a metabolite. I got pfs like symptoms from abusing steroids and crashing with anastrazole. Is the metabolite I should check also 3-ADG? All other hormonal labs are normal.

Recovering pictures from a Laptop with a stealer.

Hello guys I’m bad with tech but is there a way to get my pictures from a laptop with a stealer virus on to my Google Docs or just to my e-mail safely without having to use a usb stick or external drive? My nephew will clean the computer after that.
