Genuinely have no idea what to do. In a niche specialty, so even if there were gen med/surgical jobs going, I've been out of clinical long enough to not be able to compete for these jobs. Irish grad, went to the UK for a year after internship (only way to get experience in my specialty), and have been home for the past two years. My wife (also a doctor) is struggling to find a standalone too. Have applied to everything. I know no one has a solution, just needing to vent.
