-5 single dose vials of menotropins for injection
-5 single dose vials of 0.9% sodium chloride injection, usp, 2 mL
-5 Q-cap vial adapters

1 leuprolide acetate injection 14 mg/2.8 mL

8 tablets of cabergoline 0.5 mg
