Maternity Disability Application on SOLAS

I am a speech therapist at a D75 school. I am currently 36 weeks pregnant. I have had some preterm labor complications since 35 weeks that prompted my doctor to recommend I limit physical activity and stop working. I applied for maternity disability on SOLAS and provided the doctor's note with the diagnosis and prognosis. Once baby is born, I will be applying for UFT parental leave.

Even though I put my due date (6/13) on the application, the automatic end date for the disability leave generated on the application was 6/30. I wasn't able to edit this date. Also, I have a c-section scheduled for 6/9 so I know I will be giving birth earlier than my due date anyway. My payroll secretary suggested I call HR connect to make sure this is ok. When I called, the lady I spoke with basically didn't know why it did that and suggested it might be because my due date is on a Saturday so maybe I should withdraw the application and put 6/12 as the due date. Does any of this even matter though? I figure that the birth date is going to be different than my due date no matter what. I feel kind of weird putting the wrong due date when my medical documentation shows the actual due date.

I called the UFT last week who gave me completely wrong information about how to apply for maternity disability so I'm feeling a little disillusioned with everyone at the DOE/UFT right now. No one seems to have accurate information about what I should do when this seems like this must be a pretty common situation for pregnant women at the DOE. Would love any insight from anyone who has experienced this or knows about this type of leave.
