My [27F] boyfriend [26M] broke a promise about our accommodation while visiting his family in Italy, and my mental health is spiraling. Am I wrong for being miserable?

I (27F) and my boyfriend (26M) have been together for over a year. He is an incredibly supportive, kind person with a good heart.

I struggle with anxiety, depression, and suspect much of this stems from undiagnosed autism. Because of this, chaotic environments drain my energy completely.

My boyfriend is from Italy, and I am from a Nordic country. The cultural differences regarding family dynamics are quite huge. Currently, we are visiting his family. We strictly agreed beforehand that we would not stay at his parents' apartment to ensure I had a quiet place to retreat to. However, this plan fell through and we ended up staying there anyway.

The apartment houses his parents, sister, and two dogs. We are sleeping in his tiny, cluttered childhood bedroom. The bathroom situation offers zero privacy, the environment is constantly loud, and as an introvert with sensory issues, I am in a state of constant panic.

On top of this, my boyfriend is so busy helping his family with everything that he is neglecting his own health (he came here to fix his back) and our agreed boundaries, while his sister (23) contributes nothing. I feel completely miserable and guilty for feeling this way. I need advice on how to communicate my distress to him without causing a fight.

tl;dr: My boyfriend (26M) broke a promise to get separate accommodation while visiting his loud family in Italy. As an introvert (27F) with severe anxiety and sensory issues, I now have zero privacy, feel abandoned, and need advice on how to handle this situation.
