at a loss please help

Fifty-six days ago, on June 20th, I woke up with a headache. I went to a family picnic and initially felt well, but by around 4–5 p.m. I had completely lost my appetite and began feeling mildly queasy and strange when standing. That evening, I noticed some breathlessness.
The following day, I again woke with a headache and felt unusually sleepy. Around 4 p.m., I suddenly felt as though I could not remain standing upright and needed to crouch down. I went for a nap and felt somewhat out of it for the remainder of the evening. Over the next couple of days, I continued to feel sluggish, breathless, and generally unwell.
For approximately the next 22 days, the predominant issue became severe tachycardia whenever I stood up. I underwent cardiac investigations, including bloodwork, a chest X-ray, echocardiogram, and Holter monitoring. Based on the significant orthostatic tachycardia, the cardiologist diagnosed me with probable POTS.
However, on day 23, something changed dramatically. Rather than primarily experiencing tachycardia, I developed profound, flu-like weakness and an overwhelming feeling of being severely physically ill. The best way I can describe it is feeling as though I have been poisoned or am fighting the world’s worst virus. This was not simply fatigue. It was a profound systemic feeling of sickness, weakness, nausea, and having essentially no energy.
Now, on day 56, I am still experiencing this. I feel incredibly weak and unwell, with very little energy, significant nausea, and episodes of dry heaving. My quality of life has become extremely poor because I simply feel physically sick almost constantly.
My investigations to date have been extensive. I have had nine CBCs, thyroid testing including TSH, repeated electrolyte testing, ferritin, urinalysis, a chest X-ray, and CT scans of my brain, abdomen, and pelvis. I also had an 8 a.m. cortisol level, which was slightly elevated. Inflammatory/immune testing including ESR, CRP, C3, and C4 was reassuring. Despite all of these investigations, my doctors still do not have a clear explanation for why I feel so profoundly ill.
Initially, I was treated with propranolol and was subsequently switched to bisoprolol approximately a month and a half ago. The bisoprolol dose was later reduced because I felt that I was experiencing adverse effects from it. I am now also taking midodrine, but there remains considerable uncertainty about what is actually causing my symptoms.
On day 51, I underwent an upper endoscopy. They found a 5 mm clean-based prepyloric gastric ulcer, which was biopsied. I am currently waiting for the biopsy results. This finding has caused me significant fear because I rarely take Advil or other NSAIDs, I do not smoke, and I do not drink alcohol. My father was also diagnosed with gastric cancer at age 63, so waiting for the biopsy results has been particularly frightening for me.
Throughout this illness, I have made a significant effort to stay hydrated, and my electrolytes have repeatedly been checked and have not suggested that dehydration explains what is happening. I have lost approximately eight pounds over the course of the illness, largely because there were periods when I was barely able to eat. Over the past few days, my appetite and food intake have improved considerably with my mom here helping me, but I still have not been able to regain the weight.
At this point, I feel completely at a loss. It has been 56 days, and I remain profoundly weak, nauseated, and systemically unwell, with episodes of dry heaving and virtually no normal quality of life. I have undergone extensive testing, yet there is still no clear explanation for why I became sick so suddenly, why the initial presentation involved such significant orthostatic tachycardia, or why it subsequently transformed into this persistent and severe flu-like illness.
I am trying to understand what could connect all of these symptoms and, most importantly, what the appropriate next steps should be. After nearly two months of feeling this ill without an explanation, I am increasingly frightened and unsure of what to do next.i cant do this anymore. i feel so green. so ill.

doctors baffled

37 f. 5 ft 10

41 days ago, i woke up feeling tired with a headache. That day I went outside and felt I needed to crouch. What developed since then was tachycardia while standing. 23 days in, I began to feel flu like weakness and nausea. I dey heave often and have no apetite. I feel I am at deaths door and suffering immensly, both mentally and physicslly. I was given bisoporolol and told theres nothing else to be done for me. I am feeling very dark

in terms of testing ive had:
1- ER 5 times
2- 6 CBC’s with the latest being four days ago- all good. hemoglobin is 137 and ferritin 86
3- chest xray
4- normal echo
5- 72 hour holter showed increased HR when standinf (up to 150)
6- pelvic and abdomen US showed moderate fatty liver
7- morning cortisol showed 565 (normal range is 1-540)

Virtual cardiology appointment suspects POTS but doesn’t understand why i feel so sick. If anyone has any insight or if I need to pursue more testing please let me know. I am wrecked physically and emotionally. I don’t know how to go on like this.

adrenal crisis?

36 days ago began feeling weak with heart rate apiking to 160 in the morning. following tests: cbc, tsh, chest cray, abdomen/pelvic US, echogram and holter monitor. 14 days ago i began to feel incredibly weak, as though inwas losing conciousness. continued for 2 days, went to er- cbc and ecg was OK. i am consiming 3-5 g sodium just to stay awake. cardiologist 8 days ago said he belives i have pots, put me on 2.5 g bisoporolol and encouraged sodium. i continue to wake up, take meds, almost lose concoousness at 11 am, sleepy till 4-5 pm and rhen it “lifts”. my head feels achey. nightine some sweating and panic. horrific panic attacks as soon as i wake up. have lost 8 lbs in a month. no apetite. often dry heave. this is new as of 14 days ago. feel as though im on deaths doorstep, losing hope. note:was taking proporanol as needed. felt a bit sleepy but never like this. this virus like thing began 5 days before med change.

history of neck/armpit lymphnodes- followed by US for 2-3 years. last scan in january, all good.

any insight, is there any hope for me?

160 heart rate

37F. 5 ft 10. 123 lbs as of this morning. Naturally low bp. About a week ago I started having occasional episodes where I’d suddenly feel like I needed to crouch or sit down, similar to feeling faint in hot yoga. Over the week the episodes became more frequent, and in the last 1–2 days I’ve started feeling genuinely ill like I got hit by a bus. I’m extremely fatigued, sleepy, have no appetite at all lost 6 lbs, headaches, “blah” like sensation, but no fever cough. My heart rate now jumps to 130–160 when I stand or walk at times, and I feel short of breath and like I might faint, although I never actually do. Symptoms improve when I sit or lie down. Feels like if im walking, I have to sit. Morning HR highest (160 this morning) ER workup was normal (CBC, ECGs, cardiac bloodwork, thyroid, chest X-ray, oxygen). I have had this feeling in the past when walking up stairs, walking while carrying chairs, etc- but not this acute. Moving empty garbage bins caused it to spike to 144. Any insight? cleared by Er twice. on beta blocker now

not feeling well

I’m hoping someone can offer some insight because I’m honestly really scared.

I naturally have low blood pressure (usually around the 80s/60s to low 90s/60s), but I have NEVER experienced anything like this before. The only thing I can compare it to is when I had a really bad case of H1N1 about 10 years ago and felt extremely weak and wiped out. Other than that, I’ve never had episodes like this.

I don’t really have any typical sick symptoms right now. No cough, fever, vomiting, or diarrhea. I just feel extremely sleepy and fatigued, my mouth is very dry, I have almost no appetite, and I’ve had a mild headache and just generally feel unwell.

Last Saturday night I started feeling mildly breathless. Sunday morning I woke up with a bit of a headache and just felt “off.” Twice that day, while standing outside talking to my father-in-law, I suddenly felt like I needed to crouch down. My ears started throbbing and I got that “I’m about to faint” feeling. The best way I can describe it is that it feels like I’ve been standing in a hot yoga class for too long and I’m about to drop. I get this sudden urge that I NEED to sit or crouch down immediately.

Monday and Tuesday I just felt really sluggish and unwell. On Tuesday I had another episode where I got that same feeling, like I needed to sit down right away or I might faint, but I kept walking and it eventually passed.

Wednesday, I was outside helping my mom do some very light cleaning when I suddenly got that same sensation again, like I needed to sit down immediately. I checked my heart rate and it was 133. I wasn’t anxious beforehand or doing anything strenuous. I was already feeling tired and had a headache, and my heart rate stayed elevated and didn’t seem to come back down while I remained standing.

Thursday morning I came downstairs and my heart rate was 144. I got scared and went to the ER because I again had that feeling of needing to sit down and like I might pass out.

At the ER:

Resting heart rate on arrival was around 118.
ECG was normal.
CBC was normal.
Chest X-ray was normal.
Troponin (heart damage bloodwork) was normal.
Thyroid tests were normal.
Oxygen was 100% the entire time.

While lying on the monitor, my heart rate was mostly 84-90. Even small movements would bring it up to around 100.

Then I stood up and my heart rate went to 132 and stayed there. Along with the elevated heart rate, I got that same feeling again, like I needed to sit down and couldn’t comfortably remain standing.

I kept trying to explain to the doctor that I know anxiety can make your heart race, but this felt different. I wasn’t having a panic attack before these episodes started, and despite having anxiety and naturally low blood pressure, I have never had episodes where simply standing causes my heart rate to jump into the 130s and makes me feel like I need to sit down immediately.

What’s confusing is that Thursday evening, after I got home from the ER itt happened again and went to 144 and after thay i drank pickle juice and mu heart rate actually seemed completely normal for a while. My standing heart rate was around 88, which made me think maybe whatever was happening had passed.

Then today (Friday) it all started again.

This morning I got up and my heart rate was 144 again, with that same feeling that I needed to sit down immediately or I was going to faint. I got ready for work mostly sitting down and went to work. Throughout the day things seemed a little more stable, but then I’d randomly check my heart rate and it would be around 132 standing, then come back down to around 110.

Tonight it happened again. I stood up and my heart rate went back to 144 and I got that exact same sensation as this morning, like I had been standing in hot yoga for too long and needed to sit down immediately.

At the same time, something strange happened with my legs. Both of them suddenly felt really heavy and throbbing, almost like they were full of water. I laid down and that sensation passed. Now my right calf still feels a little achy or throbby when I flex it. I honestly don’t know if it’s related or if I’m hyperaware of every sensation right now, but it’s frightening me.

I also had two brief episodes today that almost felt like mini spins or sudden waves of dizziness, one of them while I was outside. Interestingly, my heart rate at one of those times was only around 107, so not all of my symptoms seem to happen only when my heart rate is very high.

Overall, I just feel incredibly fatigued, sleepy, weak, have almost no appetite, and my mouth feels very dry. I’ve been drinking lots of water and electrolytes because I thought maybe I was dehydrated, but it doesn’t seem to be making much difference.

Has anyone experienced something like this? A resting heart rate in the 80s to 90s, but then jumping into the 130s to 140s just from standing or very light activity and staying elevated, along with feeling like you need to sit down immediately or you’ll faint? Did it end up being dehydration, a virus, something like POTS, or something else?

I’m just really frightened and don’t understand what’s going on.
