Lack of sharpness outside

Hi everyone, just looking for some advice; My eyesight is relatively bad and I recently got a new prescription/glasses. I'm experiencing a lack of sharpness and some warping outside- I knew my left eye had changed and required a stronger prescription but I've gone from picture 1 to picture 2, should my cylinder and axis have changed this much?

Warning

This could be incredibly obvious but I'm not sure of the answer. I had a written warning a few years ago(I know I'm an idiot, I did the thing everyone is warned about doing), it's expired now- on job applications it states "Have you ever had a written warning?".

I take it I have to disclose that every time? What does it impact in terms of recruitment?
