Possible PATM connection: sebum, skin bacteria, VOCs, and environmental odors
I’m going strictly off my situation which maybe different for you.
I’ve been looking into whether PATM could involve abnormal skin/sebum chemistry rather than uric acid itself, and I found some interesting research.
A published PATM study measured 75 skin gases and found that people reporting PATM had higher emissions of several compounds, including hexanal, isovaleraldehyde, acetone, 2-ethyl-1-hexanol, toluene, xylenes, methyl mercaptan, ethyl mercaptan, and allyl methyl sulfide.
What’s interesting is that normal skin chemistry can also generate VOCs. Sebum and sweat provide substrates that skin organisms such as Corynebacterium, Staphylococcus, Cutibacterium, and Malassezia can metabolize. This can produce volatile fatty acids, aldehydes, and sulfur-containing compounds. Sebum can also undergo oxidation and generate VOCs without bacteria being directly responsible.
There’s another possible layer: clothing absorbs VOCs from indoor environments and also collects sebum, sweat, bacteria, and their metabolites. This could explain why some environmental odors, such as a musty building smell, can become strongly attached to clothing/hair/skin.
So one hypothesis worth investigating is:
Sebum production/composition → microbial metabolism + oxidation → altered skin VOC profile → VOCs accumulate on clothing and in enclosed spaces.
Environmental VOCs could simultaneously be absorbed by clothing and skin oils, potentially creating an even more complicated mixture.
This does not prove that skin bacteria or excess sebum causes PATM. The PATM study measured skin gases but did not simultaneously characterize the participants’ skin microbiomes, so that connection still needs to be demonstrated.
But I think this deserves more investigation because there are measurable PATM VOC abnormalities, known VOC production from sebum/skin microbes, and known VOC retention by clothing.
It would be really interesting to compare skin microbiome + sebum composition + skin GC-MS VOC measurements in the same PATM subjects. That could tell us whether these things are actually connected.
