Short cervix 2.7 cm not pregnant /recent mc post d&c

I miscarried at 17.5 weeks in my first ever pregnancy. My cervix was never measured but when I started spotting it was 2.1cm. Within days I miscarried and needed to have a d&c. They never confirmed if I had an incompetent cervix or an infection caused the mc.

After I finished my first period ~45 days after the mc, my doc had ordered an ultrasound to baseline my cervix which measured 2.7 cm. Is that short?

Any personal experiences and learnings will be helpful 🙏

How soon after a 2nd trimester loss did you TTC again?

Miscarriage cause unknown, lost my baby at 17w after 4 days of spotting/some pain - was on vaginal progesterone for the 4 days. First pregnancy with no past history. Path reports suggest chorioamnionitis but doctors couldn’t determine if an infection caused the cervix to shorten or the short cervix caused infection to move fast.

I had a d&c for a placenta part that didn’t deliver vaginally. I will be tested for APS and thrombophilia after 2 weeks of recovery.

Looking for success stories and some hope to stay motivated of a pregnancy following such an unfortunate fate 🙏

Not a success story, pls avoid if sensitive for you. Sudden loss at 17 w, dynamic cervix shortened. Looking for insights & some Qs on future planning from this kind community.

Hi All, I’m heartbroken to share that I lost my baby this week at 17 w. Just wanted to share my timeline to see if anyone has experienced a dynamic cervix changing this rapidly, and what your next steps looked like.

Caught a cold and nasal congestion last sun (May 3) and developed mild abdominal pain on wed (May 6) which I assumed as uterus size changing and focused on hydration. On sat (May 9) I did step out in the sun for an activity that just required me to sit in the sun although I had shade & kept hydrating for 4 hrs and 1 hr of driving in total. This is in US.

Sunday Afternoon (May 10): Had mild cramping and light spotting (pink/brown). Went to the ER for evaluation. Baby had a strong heartbeat but cervix had shortened to 2 cm. Cervix was closed with no funneling. This was the first time my cervix was measured. Doctors gave me vaginal progesterone and recommended pelvic rest although I took some bed rest.

Prior to this everything has been normal - nipt/nt scan, my blood tests and health (barely had nausea). I do though take livothyroxine at 81 mcg and my thyroid has been stable within range for the trimesters and monitored monthly. I was also taking baby aspirin since 12w. Diagnosis - Threatened miscarriage.

Sunday 10 pm: I had more discharge that looked like mucus plug with a clot right after taking the progesterone vaginally so I rushed back to ER. Follow-up ultrasound showed a 1.6 cm dynamic cervix (but closed). I felt like I was having rhythmic cramps and pain. Cervix was closed with no funneling on ultrasound. I was told I am ineligible for cerclage with no prior history of miscarriage and active cervix changes/cramping.

Put on bed rest, Progesterone, and Indomethacin for 48 hrs. I was discharged at 5 am on Monday morning but brown/pink spotting continued through the day.

Tuesday (May 12): Went for my first MFM appointment. He said he wasn’t concerned, progesterone should work and did a physical pelvic exam. He said there’s enough cervix and they can only perform a cerclage if I am asymptomatic (no fresh bleeding, pain, cramps for 48 hrs). He found it assuring that there was no blood on the gloves after physical exam.

He scheduled me for another intravaginal scan on Thursday.

My last indocin dosage was on tue night and I will say that the pain had subsided with this med.

Wed (My 13) - I went to see another obgyn at a hospital slightly farther from my place for a second opinion. The walking and sitting upright during transport was very uncomfortable. Her recommendation was the same, wait and watch - relax. She didn’t conduct any exam to rest my pelvic.

Right after the appointment, spotting turned into active bright red bleeding with a few distinct clots. The doctor said you will have some bleeding due to dynamic cervix just wait and hope it stabilizes and we will review in the scan tomorrow.

The spotting then turned brown /pink and that night right after taking vaginal progesterone I slept. A few hrs later I started having minor abdominal pain and later after a few hrs I started getting no abdominal but just back pain - like an electrical current from lower to upper back. I took a tylenol for pain and teiwd ro catch some sleep I could.

Thursday (May 14): The back pain continued but after a couple hrs of sleep I could take a shower feeling a bit better.
I then left for my scan, and the moment I entered the clinic my pain increased. I couldn’t sit on the bed foe the scan and stood up thinking I need to use the restroom. Things progressed rapidly, resulting in a miscarriage in the scan room. The baby just slid out but the placenta didn’t. Eventually, I underwent a D&C surgery to ensure entire placenta was removed from my uterus. I was monitored for 1 day and discharged.

Current Recovery: Bleeding is light, but dealing with a mild spinal headache (worse when upright, goes away when flat), which I'm treating with flat rest, caffeine, and hydration and a med by doctors.

The baby and placenta are with path but our baby was healthy till wed doppler scan with a steong heartbeat and moving.

My questions for the group:

  1. Has anyone had a short/dynamic cervix change this quickly after a brief period of physical activity or travel? Could you ever understand the cause?
  2. If you had a cervix under 2cm with spotting/ bleeding, did your doctor suggest a rescue cerclage immediately, or did they try the wait-and-see approach first?
  3. If you had a loss due to cervical insufficiency, what did your proactive plan look like for your next pregnancy (e.g., preventative cerclage, earlier monitoring)?
  4. When could you try again? I know I need to heal emotionally but just curious on the timings.
