Lower risk alternative to Clomipramine?

I'm considering medication as it's been months with no improvement, is there anything "lower risk" that's worked for hyperacusis?

Ideally if it's worked for anyone with loudness H also with headache features like I have.

What is the best strategy?

I declined over 3 months even while avoiding loud noise. I think it's because I was repeatedly testing sound tolerance to (low volume) TV/phone and getting aggravated.

But even since avoiding all sound that aggravates me and wearing earplugs while going out, I'm not getting any better. Is there like any way to spot signs of improvement? Do I have to take one of those extreme psychiatric drugs like Clomipramine? What if they don't work, you're just fucked?

All the guidance on this illness seems to contradict itself, no doctors know anything about it and idk what to do. Think I'd rather have cancer since at least people are supportive and understanding about that.

Hyperacusis with chronic headache?

Is hyperacusis being accompanied by headaches and head pain a standard presentation?

Digital audio and human speech hurt my head rather than my ears (I don't have much ear pain if any at all, only feelings of pressure when there's aggravating noise exposure). I don't have issues with traffic even though it's much louder (besides police/ambulance sirens and loud motorbikes).

It's been worsening for months now (even though I try to avoid noise exposure) and since 2 weeks ago my tinnitus has been worsening as well (previously it was stable for literally 11 years).

Hard to imagine this ever getting better.
