unclear wording on the "Backdating Questionnaire". Has anyone seen this?

I received the paperwork to apply for my backdated payments. The first page of the letter they sent me says "Questions 1, 2, and 3 are required. For the rest, only complete the sections that apply to your situation".

Well, only question 1 applies to my situation: I forgot. The other two questions seem to belong to the "Could not get through to DOL" section. Except that does not apply to me. I have looked through the entire form and there are no other questions numbered 2 or 3.

Do I have to try to reach a DOL person on the phone again or can anyone interpret this form for me? Has anybody else encountered this contradictory instruction? (the scraps of trash are me covering the private info).

Here is the form I need to fill out

and here are the confusing instructions (pink highlighting is my own)

Thank you all!

they told me to call them re severance then they told me they were too busy to take my call?

So I was filling out the form they mailed me on SHORT turn around to explain my severance and because i got a severance payment within 30 days of my last day of employment they told me to call the telephone claims center and. "speak to a representative right away". So I did. And i went thru the phone tree and the phone tree said they were too busy to talk to me. try again later. Meanwhile these forms are due Friday (and I only received them Monday).
What do I do?

Send the forms in even without the phone call? Give it more time to reach them and fax them last minute? How dare they demand we call then hang up on us without taking our calls?!? I feel like i'm damned if i do, damned if i don't and not sure which is the safer road to take here.

My EIN is for Trinet. Is Trinet "my employer"? And if so what is their phone number?

I worked for a small nonprofit and all my w2 list trinet as my employer (rather than the organization I work for). I will obv use that trinet EIN as the EIN for my unemployment application but what do i put as the phone number NY requires? Or should the address be an address associated with my actual employer? Thank you
