Morphine to dilauded

Thinking of asking my team to move to dilauded from morphine as it’s supposed to e stronger at small doses. Anyone have feedback on this switch?

Thinking of changing providers…

So my current PM provider is not helping much at all with “managing pain” that after full mri of spine and each level indicates bad issues in each, they have still only pushing me for more injections and more rfa’s! They just swapped out NPs for third time again in last full year there.

Mt new spine surgeon saw and heard my issues and he even stated PM should be doing more to help as I am not at strongest longer acting medication.

So question: he sent me to a new PM provider as I asked him if he knows of one that would listen to him and me and help manage my pain. So I see new place first and ask them what they can do, or do I talk and give one last chance to current provider?

Does your provider help with PT pain?

So I have done off and on PT for the past three years. Each time from my PM provider saying I need to get more flexible and sustain mobility. However, each time I tell them every month at follow-ups that I hurt more after the sessions and takes 2-3 days of resting and in bed from pain. I always tell them either lowering amount of medication and upping the MG dose or keeping lower MG dose and increase amount or going to a stronger one to help with this pain is something that I really need to help me recover and not be stuck in bed days after. They always say no, just tough it out and eventually the pain should get less.

Has anyone had this type of response from your provider? I’m currently talking to my spine surgeon about a new provider he thinks would understand more and him willing to advocate for me and since he knows the issues going on.

Updated spine version

So new surgeon appt went good. However I still am not sure why all this pain when he stared the only severe things he saw was my si joints. So he wants to do injections on those joints and we how it goes. If it does work out then SI joint fusions is next step. But the two fractured vertebrae are not severe enough, the three bone spurs in cervical levels aren’t severe and the dice levels and disc herniations with swollen ligaments on spine opposite of spinal cord are not severe enough either. The nerve damage he said nothing we can do so that pain will always be there and that’s what is causing the severe mid to upper back spasms. My left hip has a couple more years till it’s bad enough to need a replacement. So it’s a sit and wait time again. He’s going to send to one of his pain management places that will help more than my current pain management will the meds to manage my actual pain and listen to me. I hope it all works out because nothing is helping. I told him I want to be able to sleek at night as the last few nights I got 1-2hrs of sleep a night, and last week I got 1hr sleep per night for four days. He also said the muscle where I had my fusions done have deteriorated and turned into fat. So I have to just keep moving. However I told him when I try and walk more daily it just makes the pain worse and worse and no one would listen and my medication. Will see how new place does. It’s annoying but if he said things aren’t severe enough for other treatment than I have to listen. Just tired of it all now

Er visit?

Wondering if going to er when symptoms get worse is better than waiting for surgeon visit in two days to go over recent MRI findings? I’m stuck at what to do.

Popping and movement feeling

I have two fractured vertebrae in lumbar levels above my already fused L3-S1. And the fractured vertebrae are L1-L2, with Minimal disc height loss ventrally at T4-T5 through T8-T9. The past year I have started to feel movement and hear/feel popping sensations when moving certain ways.

Luckily I see my new spine and ortho surgeon next week to go over my full spine MRI’s. Had anyone else felt these symptoms? The last time I felt this was when I had no discs and vertabrae where fusions were done had collapsed discs and was rubbing endplate on endplate. So I’m little worried and freaked out again.

Also I have C5-C6 Tiny broad-based disc osteophyte. Mild facet arthrosis. Mild right neural foraminal stenosis. And I feel grinding/rubbing sensation there as well. And more issues in pelvis and hip joint issues I won’t get into. It’s like where the heck do we start!

Which Dr to have …

So is pain management the only drs to prescribe pain medication or does your spine surgeon prescribe these as well? Which is better to have as your medication provider?

Just when I thought they listened …

So after updated MRI scans shows significant pain related issues and although my pain Dr now knows I am not faking my pain levels, they got me on morphine 15mg IR every 8 hours. I asked to get on this plus a breakthrough pain medication for fast acting. However they said they will only do one pain medication at a time. I asked them bumping me up to 15mg morphine every 4-6 hours as my 7.5mg perc medication was being dosed and they said no that this was enough. And they were going to get me on another medication for constipation that helps absorb the morphine medication. Not sure what the theory is there but it’s not lasting long enough especially on days and after I have PT and being active more as they suggest I am. It was a win for me that I advocate so much for myself and yes it worked, however they put a wall up as to what they will allow me. Not sure if I should try and look for another location or what. I’m so close to getting what I need for my pain needs and comfortably living, it’s just like they still will limit me to not fully getting what my body and pain levels need. Ugh

How severe is osteoporosis in spine?

So since I have multiple compression fractures in lumbar vertebrae and ongoing spine pain with issues with severe pain for at least a decade, they want to test me for osteoporosis and see if hats causing the issues. However how serious is it if I do have osteoporosis? I was just told it runs in my family with my grandmother having it and actually passed away from being in the hospital and the nurses moving her and broke a couple of ribs that punctured her lungs and passed away.

Compression fractures and bone spurs

Hey all, so my latest mri shows two lumbar vertebrae fractures above my current lumbar fusions along with two vertebrae in neck with bone spurs. And both hips have cartilage wearing down so it’s just about bone on bone on hip joints. Not looking too good right about now. Which do you think is worse lol

Finally I was heard

So after finally getting my new imaging done to confirm the issues I am having and that the pain is real, my doc finally agreed to change my medication up. She said yes since you clearly are in pain from these issues going on. And I have done everything they have to offer, so many injections up and down the spine. Now they want to do my neck where I have bone spurs and disc herniations. Next meeting with be with actual spine surgeon.

MRI Results First Set no images yet

Ok so you might have seen my posts regarding pain that I have had in going as well previous surgeries and procedures to try and help limit pain. I just picked up my images via CD as well written report from radiologist. I see my spine Dr next week to go over Lumbar images and Cervical images. I also get tail bone/pelvis and Thoracic MRIs next Thursday and will then post inform those after.

So, I really didn’t think I would have issues with my C level vertebrae and almost canceled it. However I am lucky I kept it and got it done;

C3-C4: Tiny disc osteophyte. No significant spinal canal stenosis or neural foraminal narrowing.

C4-C5: Tiny disc osteophyte and mild right foraminal stenosis. Mild facet arthrosis. No significant spinal canal stenosis or neural foraminal narrowing.

C5-C6: Tiny broad-based disc osteophyte. Mild facet arthrosis. Mild right neural foraminal stenosis. No significant spinal canal stenosis or neural foraminal narrowing.

Here is the report from the lumbar region, There are minimal chronic compression fractures within the superior plate of L2-3.;

L3-4: There is minimal broad-based disc bulge and a small radial tear centrally the central spinal canal, lateral recesses, and

neural foramina are patent.

L2-3: There is no focal disc abnormality or significant stenosis. There is mild thickening of the ligamentum flavum.

Oh the fun.

So after three fusions and 20 injections with scs implant dating back to 2018, nothing is helping and new pain in tail bone and above fusions levels are getting worse. I get new imaging done to show the spine specialist what’s going on and my pain management I’m not making this up. I have tried so many types of medication some work some don’t, and the ones that do the dosage has to keep increasing. It annoys me they say to get into PT to get stronger yet that hurts and makes pain worse! I just want relief, even a just the weekend of meds to get relief I just want something to help! I know many are in same spot as me. Just venting. Nights getting harder to sleep and anxiety builds and makes things worse and spasms last longer and intense.
