Continued Struggles With Poor Feminization Despite Dosing Shift, Potential Metabolite Concerns, Seeking Advice

Hello again! I'm about 2 years on hrt, with unfortunately rather poor feminzation. I have been on monotherapy and have noticed some response to the CDG/Methylated B Vit/Magnesium supplement combo I see some people trying, however, not enough to actually get my transition moving in the direction I'd like.

About one month ago I reduced the amount of estrogen I was taking from 4mg of IM EV every 5 days, to 2mg. This was done due to hopefully reduce the amount of metabolites that were potentially accumulating in my system. While this reduction doesn't seemed to have meaningfully *harmed* my transition, I also haven't noticed any qualitative or quantitative shifts in feminization. I continue to be hopeful that there is a solution for why my transition has progressed how it has but I am beginning to suspect that metabolite clearance issues are not the culprit. At some point I'd like to get WGS and a DUTCH test done to get a better picture, as well as more properly examine metabolite clearance issues, but presently cannot afford them.

Does anyone have any idea what avenues might be worth exploring?

For Wait-listed People

How long did it take for you to get off the waitlist to see Dr. Powers? Would you say its worth hopping on now even if its realistically going to be a year or more before I can feasibly get to his practice? Thanks!

Increased Breast Tenderness Temporarily When Modifying Regimens, What Is Going On

Hello again, I have recently noticed a specific odd pattern when it comes to hrt efficacy and changing regimens.

For context, I've been on hrt for about 2 years and have been on a number of different regimens throughout, despite this I've had overall poor feminization, primarily minimal fat redistribution and poor breast development. However, I've noticed that when I've *changed* regimens I get a pattern that goes sort of like this

Change Regimen -> Breasts become notably more sore for a few weeks -> Soreness tapers to basically nothing

Part of me thinks this might just be tissue responding to hormonal fluctuations, but I'm uncertain. Additionally, I have a fairly constant mild pain response to pressure in my breasts, which is normal but also has been a constant for the past year. I'm mostly just trying to see if anyone else has had the issue I'm having, thanks!

Actual Efficacy of Estrogen Metabolism Supplement Stack

Hello again!

I recently made a post here and was recommended a number of supplements to support COMT and methylation, the exact stack I landed on is as follows:

- Calcium D-Glucarate: 500mg daily and nightly
- Magnesium Glycinate: 400mg nightly
- Sulforaphane Glucosinolate: 50mg daily
- Igennus Super B-Complex: Twice Daily

While I am willing to give this a fair shake, I wanted to know if anyone had actually seen notable improvements in HRT efficacy with a similar stack, especially in regards to breast size/development, as that's one of my primary motives. Thanks!

Transbian, still worth trying topical T for breast development?

Hi all. I'm about two years into my transition, and my breast development has been stalled at around a tanner 4 for nearly a year. I'm on 4mg of IM EV every 5 days and 200mg of rectal progesterone nightly. My levels are:
- 281 pg/ml E
- 16 ng/dl T
I know that Dr. Powers sometimes uses topical testosterone to unstall late stage development, but in one of his posts on the matter (here) he mentions that it is unlikely to work for bi/transbians. I am very much a transbian and wanted to know if this approach is still worth trying.

Hello!

I'm 21 and have been on hrt for about 2 years now, and in many ways I've had good results. My body hair has thinned out, my skin has changed completely, I'm at a tanner 4 breast development stage and I've gotten some other minor feminizing effects as well as substantial emotional changes.

However, I have had a total lack of fat redistribution, I lose weight like normal, but when I gain weight it goes completely to central fat stores like my stomach. This has left me somewhat disappointed, as despite having gotten many effects, I'm still quite unhappy with the shape of my body. This has also left me confused.

I'm on 2mg of IM EV every 3 days, 200 mg of progesterone nightly. My levels look great (E trough ~300, T ~16, SHBG < 100, no issues with DHT, FSH, LH or anything of the sort). I am beginning to suspect insulin resistance as a possible cause, I admittedly am pretty sedentary, and have pinged for high glucose and triglycerides consistently.

My question is: Is insulin resistance a possible reason I'm not getting gynoid fat distribution, and if so would it be worth trying something like pio or metformin (in addition to lifestyle changes)?
