Experiencing foot pain after switching from cages to TIEMs

Hey everyone,

I’ve been spinning regularly for about 4 months now at Equinox and absolutely love it. I recently made the upgrade from regular sneakers in the pedal cages to a pair of TIEM Slipstreams with SPD cleats, and the difference in the workout is amazing.

However, I’ve been having some foot pain after riding in them, towards inside of foot , and it usually lingers for about a day.
When I first got them, I had REI install the cleats, and they set them up in what they called a "standard/middle" position. But given the discomfort, I think the alignment is off for my specific stride, and I don't totally trust a general outdoor store to get the fine-tuning right for a spin bike. My instructor actually recommended I take them to a dedicated local bike shop to get them properly adjusted.

Before I do, I wanted to check in here:
* Has anyone else experienced foot pain or a transition period when switching from cages to clip-in spin shoes?
* If you ride in TIEMs, where do you find your sweet spot is for cleat placement (forward, back, angled)?
* Any tips on what I should ask the bike shop to look for when they check my alignment?

Hi everyone, new here. I’m a 57F and just coming out of a brutal 3.5-day episode. My pain was epigastric (centered right under the breastbone) and the sensation was very specific: it felt like something was being inflated under high pressure and then slowly deflating, over and over like labor contractions.

The pain also radiated straight through to my back. This went on consistently for over 80 hours. It finally stopped around 3:00 AM last night, but only after I did a total fast all day yesterday. I did feel feverish last night at the tail end of it, and I have zero hunger cues today.

I haven't had any jaundice or dark urine, but since this is my first time experiencing anything like this, I'm trying to figure out if this 'inflation' feeling and the long duration are common. For those who had these 'marathon' attacks, did you find they were a one-off, or was this the sign that the gallbladder had to come out? Also, how did you handle the transition back to solid foods without triggering a 'rebound' attack?
