Has anyone else had truly treatment-resistant acne?

I’m 28 and I’ve honestly spent the last half of my twenties battling acne. It’s affecting my confidence so much, and I’m starting to feel like I’ve run out of options.
So far I’ve had:
3 courses of Accutane (isotretinoin)
Consistent topical treatment with Aklief
1 month on 100 mg spironolactone (started at a lower dose before that)
Tried simplifying my skincare and even addressing possible gut issues
I still get deep, painful nodules that seem to take weeks or months to heal, and some never seem to fully go away. I’m also left with a lot of persistent redness and marks.
I know one month on 100 mg spironolactone is still early, but after everything I’ve already tried, it’s hard to stay hopeful.
Has anyone been in a similar situation and eventually found something that worked? Did spironolactone take longer to kick in for you, or did you end up needing a fourth course of Accutane or another treatment?
I’d really appreciate hearing from people who’ve been through something similar because I’m struggling to believe my skin will ever get under control.
