dont know if i should take me meds tonight or not: biggest exam of my life tomorrow

i take sertraline in luteal only normally for 6-11 day before. im 8 days out and i can REALLY feel it starting. (i was on the train this morning and i realised i had decided every single person on the carriage was evil and so annoying and i spent the whole journey thinking about how much i hated them... they were random strangers who had done nothing wrong!) so normally i would take my meds tonight and feel a fair amount better tomorrow. but the problem is sometimes (not always) the meds make me shaky/dizzy on the first day of each time i take them. and i have the most important exam ive ever sat tomorrow!!!

if i don't take my meds:

- ill be in a terrible mood all day, probs accidentally be a pretty rude to anyone who tries to talk to me

- ill might be super sensitive to sound (happens to me during luteal) which will make me easily distracted and irritable in the exam

- if i get stuck on a question, ill probs start crying, maybe have a panic attack, and mess up the whole paper. if i do take them i should be able to remain calm even if it's a hard exam

but if i DO take them:

- i might not sleep tonight

- i might feel quite nauseous

- i might be very shaky

- my heart sometimes beats faster than usual on the 1st day which could make my exam anxiety worse

anyway i keep changing my mind can someone tell me what they would do lol?

this article was in the guardian (one of the biggest newspapers in the uk) today. i thought it was a great read tbh, it doesn't sanitise its description of our suffering in any way, and i think one of the interviewees made a great point that PMDD is much better understood by people who have it than by doctors. ive got so used to that, that i forgot that your doctor is actually meant to know more about your condition than you do lol.

anyway it got me thinking that discussion of this disorder really is at an all-time high- just a few days ago some people posted an article from the national geographic, and i even feel like more of the people i talk to about it in my social circles have heard of pmdd before.

this condition is shit, but hey, at least more people are finding out it exists!

I've been taking sertraline during my luteal phase for a couple of cycles and it works great!!

However I would really love to understand the science better. I keep reading about 'upregulation of allopregnalone' but I don't really get what that means! Are people with PMDD too sensitive to allopregnalone or not sensitive enough? How do SSRIs combat that? I'm just confused how it can work like magic for so many PMDDers, but takes ages to start doing anything for other mental health conditions.

Google hasn't been that helpful, I feel like I'm enough of a woman of science to want to know, but not enough to understand all the scientific journals lol. I would really like someone who understands to try and translate into layman's terms.
