Is it really close to impossible for a first gen doctor to “make it”?

So I was watching this clip of Dr Pal Manickam (who’s a famous Indian influencer doctor who now practices in USA) say that he dares any first generation doctor to say that he will let his son become a doctor in India because it’s impossible to make it for first gen doctors.

I really wanted to ask is this true? Obviously the “make it” part is subjective. And I know struggle is there even a few years post PG but afterwards is it really impossible now to make a good salary as a doctor who’s first gen, willing to work hard but not a genius ?

For context I’m an MBBS intern who’s first gen, and I come from a well off family so I don’t worry THAT much about getting a medicine PG seat (since my parents have enough savings to easily afford private) and do intend to do SS since I want to work in a tier 1 city.

I don’t deny that PG and life after PG is better abroad for work life balance and salary reasons but I don’t want to go abroad for personal reasons. And I know life during PG is insanelyyy hectic and pay is awful. But I really want to know what’s the case afterwards.

But I really do want to know this especially from people who have done PG. also I do know some places like Kerala and Tamil Nadu for some reason pay like shit but I don’t intend to practice

Really confused about this because some comments were agreeing with him but also I know a lot of specialists here who seem to be making a good salary after some years of experience

Also by good salary I do mean 50lpa + after 10 years after SS in medicine branches in tier 1 cities
